DO YOU TAKE MY INSURANCE?

At CROSSOVER, we are considered “out-of-network” providers. This allows us to treat the human in front of us and not the diagnostic code. Unfortunately, the current in-network model of physical therapy doesn’t allow for the individual attention and detailed treatment that is provided by our Doctors of Physical Therapy at CROSSOVER. Here you will receive the high end, one-on-one services that you deserve. We do accept FSA/HSA, BUT we do provide superbills for people with out-of-network benefits.

WHAT IS A SUPERBILL?

A superbill is a medical receipt that we will provide you when you pay upfront with us after your session. You can use this receipt to personally file for reimbursement from your insurance company to offset some of the out-of-pocket expense. Many out-of-network benefits reimburse a part, if not all of the services billed once your deductible has been met, this is not a guarantee but it is always good to check your benefits.

DO I NEED A PRESCRIPTION OR REFERRAL FROM MY DOCTOR?

NO! In the state of Florida we have direct access for 30 days, meaning you can see a physical therapist and progress through your plan of care for 30 days without a referral. Our Doctor’s advanced training allows them to diagnose and treat based on impairments but also the knowledge to refer to a licensed medical practitioner if the condition lies outside their scope of physical therapy practice. We specialize in being the first point of care for patients suffering from acute or chronic pain issues and we are trained in screening for pathology that may not make you amenable to physical therapy. We are trained to assess the whole body and can begin helping to correct your problem immediately. This makes it very easy for patients to begin feeling better sooner, as they are not subjected to the lengthy process of being passed around from one specialist to another.

CANCELLATION AND NO SHOW POLICY

We require at least 24-hour notice if you need to cancel your appointment. For cancellations under 24 hours there will be a fee. No-show appointments will be charged the full price of the service. Extenuating circumstances and special situations will be reviewed on an individual basis.

WHERE ARE SESSIONS CONDUCTED?

CROSSOVER is located inside Alpha Omega Fitness at 4424 N Lois Ave #1, Tampa, FL 33614. Here is a video to help you find where to park and how to enter the facility.

WHAT SHOULD I EXPECT FOR MY FIRST VISIT?

Your first visit will include a thorough full body evaluation, movement assessment, hands on treatment and education for you to gain a better understanding of your body and how you move. During this time, your Doctor will get a better understanding of you as a person and your personal goals.

HOW ABOUT FOLLOW UP APPOINTMENTS?

Follow up appointments will be based on findings during your initial evaluation and your need. Because of our cash model, we are awarded the flexibility to cater to your individual needs and goals. Sessions will include a combination of manual therapy body work, tailored exercises and strategies for home management. Most sessions are 1x/wk but will be discussed with your provider based on your goals. The majority of clients feel better, move better, and are ready to perform within 10 sessions.

HOW LONG DO YOUR SESSIONS TYPICALLY LAST?

Your evaluation and follow-up sessions will all be 1-hour long and will be one-on-one with your sports physical therapist.

HOW IS YOUR PHYSICAL THERAPY DIFFERENT FROM OTHER COMPANIES?

We look at the site of the symptoms as well as the whole body because we believe that everything is connected. Our approach is holistic and all-inclusive because we know that optimizing movement in order to relieve you of pain and dysfunction requires an approach that takes everything into consideration.

You will leave with an exact plan of how it relates to your story and how you will progress to your goals. Your whole session will be spent one-on-one with a physical therapist instead of on your own or with someone who is not qualified to guide you through the process. We never double-book or overlap appointments. We listen to you and provide what you need to reach your goals. Your entire experience is centered on you. This leads to long term success.

IT LOOKS LIKE YOU ONLY TREAT ATHLETES & I DON’T CONSIDER MYSELF AN ATHLETE OR WORK OUT OFTEN, CAN YOU STILL HELP ME?

If you have a body and are active, then you’re an athlete! We specialize in athletes and treat all ages and experience levels. Some examples include youth athletes, those new to working out, experienced competitors, weekend warriors, active moms, active dads, and even active older adults. We meet you where you’re at and come up with a game plan together that works toward achieving your goals, no matter what they are!

DO YOU OFFER VIRTUAL SESSIONS?

Yes, you will receive the same quality of care and training from the comfort of your own home. Whether you are out-of-state or traveling for work, telehealth allows us to make sure you continue to progress through your prescribed plan of care.

WHAT DO I NEED TO BRING FOR MY FIRST SESSION?

We are movement based so we require you to bring any sort of clothing that you feel comfortable moving in. If you have a knee problem, it is best to wear shorts. For a shoulder problem, a tank top is a good option. For low back problems, wear a loose-fitting shirt and pants.

WHAT SORT OF TRAINING DOES YOUR THERAPIST HAVE?

All of our physical therapists are doctorate trained and certified as Athletic Trainers or strength coaches. Everyone has taken numerous courses, continuing education, and have been mentored by some of the industry elites.
