- Patellar Tendinopathy (aka Jumper’s Knee or Runner’s Knee): This is the big one. It’s caused by being underprepared for a repetitive activity and shows up as pain right below the kneecap. It’s not just tendon inflammation—it’s actual breakdown of the tendon collagen over time.
- Patella Tendon Tears or Ruptures: Less common, but much more serious. Surgical repair is typically required. This is when the tendon actually tears—usually from a sudden, forceful movement like an awkward jump or being tackled down while the knee is fully bent.
- Tendinosis: This happens when tendinopathy is left untreated for too long. It’s a chronic state where the tendon is no longer healing and becomes degenerative.
How Do These Injuries Happen?
The majority of patella tendon injuries are a result of being underprepared to a repetitive stress —but there are a few key risk factors:
- Doing Too Much, Too Soon: Returning to sport after time off, or suddenly ramping up intensity, volume, or frequency.
- High-Velocity Forces: Think getting tackled in football, taking off or landing hard from a jump—especially when your knee is fully bent.
- Limited Joint Mobility: Joint stiffness in the hips, knees, or ankles can shift extra stress onto the patella tendon.

- Gradual Return-to-Sport Program: Don’t rush. Build up confidence, strength, power, jumping, running, and cutting movements with a plan, not guesswork.

- Mobility and Stretching Work: Especially for your hips, knees, and ankles.
